At Headscape we have been using project risk assessment for a little while now and are really reaping the benefits. I started to discuss it myself on a recent blog post.&lt;/p&gt;&lt;p&gt;It can take  a little time to do properly but it's definitely time well spent.&lt;/p&gt;&lt;p&gt;I really like the diagram that you have included above.
